by reporting suspected child abuse

Completing the CY47

If you are an Authorized Adult, and therefore a mandated reporter, you must also complete a written report of suspected child abuse. This report is called a CY47 and must be completed within 48 hours of the verbal report being made. After you fill out the form, you must deliver or mail it to the local county Child Protective Services (CPS) agency. The form can be accessed online or in person at your county CPS agency.

The written report includes the following information, if available:

  1. The names and addresses of the child and parents/guardians
  2. Where the alleged abuse occurred
  3. The age/gender of the alleged victim
  4. The nature and extent of the suspected abuse
  5. The name and relationship of the alleged abuser
  6. Family composition
  7. The source of the report
  8. The person making the report and contact information
  9. The actions taken by the reporting entity
  10. Any other relevant information
